	This manual is to be used to conduct the smoking cessation intervention with your patients and implement the program in your office setting. Curriculum Coordinates with the slide presentations and provides expanded text for additional reading. Clean Air is tracking changes; please forward any changes you may find to icburchette@paaap.org. Appendix A Case Studies Use these case studies to become familiar with using the 5 A's in combination with the program materials as part of your counseling intervention Appendix B Patient Materials These materials can be used to assist you as you conduct a smoking cessation intervention with your patient. 	TRAINER FORMS (LEVEL II MANUAL)


	These forms are to be used by certified Clean Air trainers. Checklist A list of duties and responsibilities you should complete before, during and following a training. Disclosure Form A Yellow UPMC Disclosure Form for Continuing Medical Education Credits that MUST be created and present at the time of the training to qualify for CME/CEU credit. Clean Air will create the Disclosure form form you. Initial Follow-Up Form For use when conducting follow-up with providers at 2, 6, and 12 months. Office Implementation Plan This worksheet assists the office manager in assigning tasks to staff when implementing the intervention.
